(A)In addition to the attachments specified in division (B)(3)(a) of section3702.304of the Revised Code, a variance application must contain or include as attachments, for each consulting physician described in division (B)(2) of that section, a signed statement in which the physician attests to both of the following:
(1)The physician does not teach or provide instruction, directly or indirectly, at a medical school or osteopathic medical school affiliated with a state university or college as defined in section3345.12of the Revised Code, any state hospital, or other public institution.
