(A)With regard to a permit issued under section1521.29of the Revised Code, the permittee shall submit to the chief of the division of water resources an annual report containing any information as the chief shall require by rule.

(B)If the facility for which a permit has been issued under section1521.29of the Revised Code withdraws ground water, the chief may require the continued monitoring and reporting of water levels in each aquifer via existing wells or new monitoring wells drilled by the permittee.

(C)With regard to a permit issued under section1521.29of the Revised Code, the permittee, at least once every five years, shall certify to the chief that the facility for which the permit has been issued is in compliance with the permit.
