§ 18-a. Disclosure of information.

1.Each health care professional\nlicensed, registered or certified pursuant to title eight of the\neducation law shall, upon request, provide to his or her patient or\nprospective patient the following:\n (a) information related to the health care professional's educational\nbackground, experience, training, specialty, and board certification, if\napplicable;\n (b) affiliation with any health care facility licensed pursuant to\narticle twenty-eight, thirty-six or forty-four of this chapter, and any\nfacility licensed pursuant to article nineteen, twenty-three, thirty-one\nor thirty-two of the mental hygiene law;\n (c) information regarding the health care professional's participation\nin continuing education programs and compliance with any licensur
