§ 548-d. Application for funding.
1.Application process. In response\nto a request for proposals, an eligible applicant may submit an\napplication for funding for a project to the department, which shall\nreceive all such applications on behalf of the committee. The department\nshall forward all such applications for funding to the committee for\nreview and approval.\n 2. Components of application. Each application for funding shall\ninclude, but not be limited to:\n a. a description of the geographic boundaries and demographic\ncharacteristics of the neighborhood in which the proposed project\nservices will be targeted and an explanation of why that neighborhood\nwas selected;\n b. a description of the services that currently exist in the targeted\nneighborhood, any gaps in or barri

Each application for funding shall\ninclude, but not be limited to:\n a. a description of the geographic boundaries and demographic\ncharacteristics of the neighborhood in which the proposed project\nservices will be targeted and an explanation of why that neighborhood\nwas selected;\n b. a description of the services that currently exist in the targeted\nneighborhood, any gaps in or barriers to such services, including\nidentification of any known statutory or regulatory barriers, and the\nresources available within the community to support such services;\n c. a description of how existing services will be made more accessible\nto residents of the targeted neighborhoods, whether or not any existing\nservices will be expanded, what new services will be provided, how both\nthe new and existing services will be coordinated, and what links will\nbe established among the providers of services in the community;\n d. a statement of the project's goals and objectives, including the\nintended outcome for each specific service or activity contained in the\naction plan provided, however, that such outcomes may include, but need\nnot be limited to, appropriate measurable factors relating to\nemployment, health, education, substance abuse, and alcoholism;\n e. a description of the process and criteria that will be used by the\napplicant to select members of the council;\n f. a description of the method that will be used to develop the action\nplan. Such description shall address the process to be used to engage\nthe community residents in developing such action plan; and\n g. a description of the community's involvement in the development of\nthe application including the involvement of local governmental\nagencies, voluntary agencies, businesses, foundations, community\norganizations, and community residents, and the methods by which the\napplicant will assure continued community involvement in the delivery of\nservices and long range planning efforts.\n
