§ 504. Content of claim, or notice of appearance. The claim, or notice\nof appearance shall include:\n (A) the name and post office address of the condemnee;\n (B) reasonable identification by reference to the acquisition map or\notherwise, of the property affected by the acquisition, and the\ncondemnee's interest therein;\n (C) a general statement of the nature and type of damages claimed,\nincluding a schedule of fixture items which comprise part or all of the\ndamages claimed;\n (D) if represented by an attorney, the name of the condemnee's\nattorney and his office and post office address and telephone number\nsubscribed at the end of the claim.\n The claims and copies thereof, may be either typewritten, printed, or\nphotocopies of the original.\n
