Text

The license when issued shall state: A. that it is issued pursuant to the Collection Agency Regulatory Act and the rules and regulations established thereunder and that the licensee is duly authorized to conduct business under the Collection Agency Regulatory Act; B. the names of the owners of the licensee, if a sole proprietorship or partnership; and if a corporation, the name shall be followed by the words "a corporation"; C. the name under which the licensee is to operate; D. the location by street number, city, county and state where the licensee is to conduct business; and E. the number and the date of the license.
