Text

A. The clearinghouse shall distribute missing person report forms to law enforcement agencies in the state. B. A missing person report may be made to a law enforcement agency in person, or by telephone, electronic media or other indirect method of communication and the person taking the report may enter the information on the form for the reporter. A missing person report form may be completed by the reporter and delivered to a law enforcement officer. C. A copy of the missing person report form shall be filed with the clearinghouse. D. A missing person report form shall include, to the extent available, the following information:

(1)the missing person's:
(a)name, including any alternative names used;
(b)date of birth;
(c)identifying marks, including birthmarks, moles, tattoos and scar
