Text

I. A copy of the order appointing the guardian shall be given in hand or mailed by first class mail to the ward and the ward's counsel. The court may order service by other means if it deems appropriate. The order shall contain the name and address of the guardian. The order shall also contain notice of the ward's right to appeal the guardianship appointment and of his or her right to seek alteration or termination of the guardianship at any time. II. Letters of guardianship shall contain:

(a)The name, address and telephone number of the guardian of the person and estate, or the person, or the estate;
(b)The name, address and telephone number of the ward;
(c)The nature and scope of the guardianship, whether over the person and estate, or the person, or the estate;
(d)The specific legal
