Text

I. Each licensee shall submit a report of authorized delegates within 45 days of any change to the number of authorized delegates. The commissioner is authorized and encouraged to utilize NMLS for the submission of the report required by this section provided that such functionality is consistent with the requirements of this section. II. The authorized delegate report shall include, at a minimum, each authorized delegate's:

(a)Company legal name;
(b)Taxpayer employer identification number;
(c)Principal provider identifier;
(d)Physical address;
(e)Mailing address;
(f)Any business conducted in other states;
(g)Any fictitious or trade name;
(h)Contact person name, phone number, and email;
(i)Start date as licensee's authorized delegate;
(j)End date acting as licensee's authorized d
