Text

All licensed physicians practicing ophthalmology in this state shall report, with the permission of the patient, all cases of vision examination results of 20/200 or less, in the better eye, after correction, to the blind services program, bureau of vocational rehabilitation, department of education. Such report shall contain the name and address of the examined individual, date of birth, the amount of vision in both eyes, and the cause of visual impairment. The information contained in said report shall be treated as confidential by the bureau.
