Text

All licensed optometrists in this state shall report, with the permission of the patient or the patient's parent or guardian, all cases of vision examination results of 20/200 or less in the better eye, after correction, to the division of workforce innovation, department of education. Such report shall contain the name and address of the examined individual, date of birth, the amount of vision in both eyes, and the cause of visual impairment. The information contained in the report shall be treated as confidential by the division.
