Text

I.Applications for licensure to practice dentistry shall be made to the office of professional licensure and certification in writing or online and shall be accompanied by a fee established in rules adopted under RSA 541-A by the office and by satisfactory proof that the applicant is a graduate of a school that is recognized by the Commission on Dental Accreditation (CODA) and holds the degree of D.D.S. or D.M.D. The applicant shall be of good professional character and 18 years of age or older.
II.Any person applying for any license or privilege under this chapter, including any person seeking to convert from inactive to active status, shall provide the office with information relating to dental competence and professional conduct, to permit the office, based on criteria established by
