Text

I. All license applications shall be made on the forms supplied by the lottery commission and shall be submitted to the lottery commission by the individual applicant or an officer, director, or duly authorized official of the entity seeking a license. II. Proof of authority to submit the application on behalf of the entity may be required by the lottery commission. III. All applications shall include:

(a)The type of license for which applied.
(b)The name, address, and telephone number of the applicant.
(c)A criminal history record release form, as provided by the New Hampshire division of state police, department of safety, which authorizes the release of his or her criminal history record, if any, to the lottery commission. The requirements of this subparagraph shall not apply to char
