Text

A person who loses or by mistake or accident destroys his license may, upon payment of a fee of $5.00, receive a duplicate license if the applicant completes affidavits setting forth the circumstances surrounding its loss or destruction. The duplicate license shall entitle the licensee to engage in all activities permitted on the original license.
