Text

1. The work certificate shall show

(1)The age, sex, place and date of birth and place of residence of the child;

(2)The name and place of residence of the child's parent, guardian or custodian;

(3)The name and address of the employer; and

(4)The nature of the employment for which the work certificate is issued. 2. Work certificates authorizing the employment of a child during nonschool hours shall be of a form and color distinct from those authorizing full time employment.
