Text

Every operator shall submit a report to the director during the month of January of each year, which report shall include the name of the person or firm engaged in mining, the location of the mines, the number of acres of land owned or leased, the number of shafts, adits, slopes or open pits operated, the number of men employed in and about the mines, average wages paid employees, the amount of mineral produced, the number and description of all accidents resulting in serious personal injury and the number and description of all fatal accidents.
