Subdivision 1.Requirement. Every health maintenance organization enrollee residing in this state is entitled to evidence of coverage or contract. The health maintenance organization or its designated representative shall issue the evidence of coverage or contract. Subd. 2.Filing with commissioner. No evidence of coverage or contract, or amendment thereto shall be issued or delivered to any person in this state until a copy of the form of the evidence of coverage or contract or amendment thereto has been filed with the commissioner of health pursuant to section62D.03or62D.08. Subd. 3.Required provisions. Contracts and evidences of coverage shall contain:

(1)no provisions or statements which are unjust, unfair, inequitable, misleading, deceptive, or which are untrue, misleading, or decep
