§ 22 §822 — Reporting

1.Notification by physician. Whenever a physician knows or has reason to believe that a person whom the physician examines or cares for has a disease or condition designated as notifiable, that physician shall notify the department and make such a report as may be required by rules of the department. Reports must be in the form and content prescribed by the department and the department shall provide forms for making required reports.

The department may require a designated health care facility, as defined in section 802, subsection 4‑A, paragraph A, to report information about its emergency management plans and operations. Reports must be in the form and content prescribed by the department and the department shall provide forms for making required reports.

Reporting by health care facilities.
The department may require a designated health care facility, as defined in section 802, subsection 4‑A, paragraph A, to report information about its emergency management plans and operations. The department also may require a designated health care facility to report other information, including, but not limited to, daily reporting of the number of available beds within that facility providing residential or inpatient services and for the reporting to be done through an online database approved by the department. The department may adopt rules that designate further information required for reporting emergency plans. Rules adopted pursuant to this subsection are routine technical rules as described in Title 5, chapter 375, subchapter 2‑A.
