Section 177. An application for original certification and licensing, renewal certification and licensing, and examination shall be made in writing to the board on forms approved by the board.An appropriate fee, as fixed pursuant to section one hundred and seventy-six, shall accompany an application for original certification and licensing, renewal certification and licensing and examination.
