(a)Every report or statement made under the campaign finance act shall be made on forms prescribed by the commission, and contain substantially the following: "I declare that this (report) (statement), including any accompanying schedules and statements, has been examined by me and to the best of my knowledge and belief is true, correct and complete. I understand that the intentional failure to file this document or intentionally filing a false document is a class A misdemeanor." ____________________ _____________________ (Date) (Signature) Every report or statement shall be dated and signed by:
