Text
(a)An approved eligible program provider
shall conduct a survey of the individuals:
(1)who participated in a course or certification or other program
of the approved eligible program provider; and
(2)for whom the approved eligible program provider received
reimbursement under this chapter.
(b)The survey shall be conducted in the year after the year in which
the individual graduates or is no longer enrolled with the approved
eligible program provider and the next four (4) consecutively
succeeding years.
(c)The survey must include the individual's employment status,
including whether the individual is employed full-time or part-time, for
each year the survey is conducted.
(d)The approved eligible program provider shall submit each
survey conducted under this section to the department n
