5.

(a)An operator shall have, in a fireproof repository located in an area on the surface of the mine chosen by the operator to minimize the danger of destruction by fire or other hazard, an accurate and up-to-date map of the mine drawn on scale. The map shall show:
(1)the active workings;
(2)all pillared, worked out, and abandoned areas, except as provided in this section;
(3)entries and aircourses with the direction of airflow indicated by arrows;
(4)contour lines of all elevations;
(5)elevations of all mine and cross or side entries;
(6)dip of the coalbed;
(7)escapeways;
(8)adjacent mine workings within one thousand (1,000) feet;
(9)mines above or below;
(10)water pools above;
(11)either producing or abandoned oil and gas wells located within five hundred (500) feet of such
