§ 16-52-2-1 — Registry; application; required information; fee; waiver
(a)The state department shall establish a
temporary health care services agency registry.
(b)The state department shall develop an application for the registry
that at least requires the applicant to provide the following:
(1)The name and addresses of each person that has an ownership
interest in the temporary health care services agency.
(2)If the owner is a corporation or other business entity:
(A)the name of the state in which the business is incorporated,
a copy of the articles of incorporation or association or similar
documentation, and any amendments or bylaws;
(B)the location of its principal place of business; and
(C)the names and addresses of its directors and officers.
(3)A schedule or description of all fees, charges, or commissions
that the temporary health care servic
