Text

(a)An organization or entity that establishes and administers a program shall do the following:
(1)Subject to section 6 of this chapter, establish guidelines for financial eligibility for the program, which must be based on financial need.
(2)Collaborate with health care providers to notify a prospective participant about the program.
(3)Provide written notice to a prospective participant on the:
(A)requirements described in this section;
(B)nature and availability of financial support provided under the program; and
(C)program's financial eligibility guidelines.
(4)Receive and review applications for participation in the program.
(5)Establish a process for awarding payments.
(6)Subject to approval by the organization or entity, provide payment to a participant for reasonable cos
