§ 16-41-14-12 — Donor information
Indiana·Title 16 HEALTH·Art. 41 PUBLIC HEALTH MEASURES FOR THE·Ch. 14 Communicable Disease: Testing of Semen Used in
(a)A practitioner shall require a semen
donor to provide the following information:
(1)Name.
(2)Address.
(3)Date of birth.
(b)A practitioner shall request a semen donor to provide the semen
donor's Social Security number.
