Text
(a)Both the state registrar and the local health
officer shall maintain a permanent record, accompanying the birth
records, that contains for each request for a certified copy of a birth
certificate the following:
(1)The date of the request.
(2)The name and address of the person making the request.
(b)The permanent record for each request required under this
section may be maintained in the Indiana birth registration system
(IBRS).
