§ 16-36-7-30 — Resources and sample forms
(a)The state department shall maintain a list
of resources on its Internet web site, including sample advance
directive forms that are consistent with this chapter.
(b)A declarant is not required to use any official or unofficial form
to prepare and sign a valid advance directive.
