§ 16-28-2-2 — Applications

Indiana·Title 16 HEALTH·Art. 28 HEALTH FACILITIES·Ch. 2 Licensure of Health Facilities

To obtain a license, an applicant must submit
an application on the prescribed form showing that the applicant is of
reputable and responsible character and able to comply with the
standards for health facilities established by this chapter and rules
adopted under this chapter.
