(a)When a permit has been suspended due to
a violation of any of the bacterial, coliform, somatic cell, or cooling
temperature standards, the board shall, not later than one (1) week after
the receipt of a written application for reinstatement of a permit, issue
a temporary permit after determining by an inspection of the facilities
and operating methods that the conditions responsible for the violation
have been corrected. Samples must then be taken at the rate of not
more than two (2) per week on separate days within a three (3) week
period, and the board shall reinstate the permit upon compliance with
the appropriate standard, as determined in accordance with section 13
of this chapter.
