Text

To obtain a license required under this chapter, a person must file an application for the license with the state veterinarian. The application must include the following information:

(1)The name and address of the applicant.
(2)The location of the place of business.
(3)The number and location of all substations.
(4)The number and kind of vehicles to be used.
(5)Any other information required under this article or by rules adopted by the board. The application must be accompanied by the license fees required under this chapter. [Pre-2008 Recodification Citation: 15-2.1-16-3.]
