§ 15-16-3-8 — Application for license; fee

(a) To obtain a license described under section
7 of this chapter, a person must:
(1) submit an application; and
(2) pay a fee of one hundred dollars ($100) for each location from
which the person distributes tank mixed liquid fertilizer:
(A) in Indiana; and
(B) outside Indiana that conducts operations in Indiana;
to the state chemist.
(b) An application for a license issued under this chapter must
include:
(1) the name and address of the person applying for the license;
or
(2) if the application submitted applies to at least one (1) location
described under subsection (a)(2), the name and address of each
location.
