(a)During April of every other year, each
association organized or reorganized under this chapter shall prepare
and file a biennial report setting forth the following information:
(1)The name of the association and the state or country under
whose law the association is incorporated.
(2)The address of the association's registered office and the name
of the association's registered agent at that office in Indiana.
(3)The address of the association's principal office.
(4)The names and business or residence addresses of the
association's directors, secretary, and highest executive officer.
(b)The biennial report of each association must be:
(1)made on a form prescribed and furnished by the secretary of
state;
(2)signed by any current officer of the association or, if the
