§ 1008 — Applications
Illinois·Topic REGULATION·Ch. 225 PROFESSIONS, OCCUPATIONS, AND BUSINESS OPERATIONS·Act 225 ILCS 230/ Solid Waste Site Operator Certification Law.
Applications for certification shall:
(a)Be submitted by natural persons on forms prescribed by the Agency;
(b)Contain statements under oath showing applicant's education and experience;
(c)Contain not less than 3 references; and (d) Be accompanied by the proper fee.
