Text
A parent may request a cost-of-living alteration by submitting all of the following to child support services:
1.Awrittenrequestforacost-of-livingalterationtothesupportordersignedbytheparent
making the request.
2.Astatementsignedbythenonrequestingparentagreeingtothecost-of-livingalteration
to the support order.
3.A statement signed by each parent waiving that parent’s right to personal service and
accepting service by regular mail.

4.Other documentation specified by rule of the department.
