Text
Each health or social agency, attending or consulting physician, or attending or consulting nurse shall report, in writing, to the Division the name, age, and residence of each person with blindness or person with visual impairment to whom the agency, physician, or nurse provides service, and furnish additional information that the Division requests for the registration or prevention of blindness or visual impairment.
