(a) Any person who may be eligible for compensation pursuant to sections 54-201 to 54-218, inclusive, may make application therefor to the Office of Victim Services. If the person entitled to make application is a minor or a person who lacks capacity, the application may be made on such person's behalf by a parent, guardian or other legal representative of the minor or person who lacks capacity. (b) In order to be eligible for compensation services under sections 54-201 to 54-218 , inclusive, the applicant shall, prior to a determination on any application made pursuant to sections 54-201 to 54-218 , inclusive, submit reports if reasonably available from all physicians, surgeons, physician assistants, advanced practice registered nurses or mental health professionals who have treated or examined the victim in relation to the injury for which compensation is claimed at the time of or subsequent to the victim's injury or death. If in the opinion of the Office of Victim Services or, on review, a victim compensation commissioner, reports on the previous medical history of the victim, examination of the injured victim and a report thereon or a report on the cause of death of the victim by an impartial medical expert would be of material aid to its just determination, said office or commissioner shall order such reports and examinations. Any information received which is confidential in accordance with any provision of the general statutes shall remain confidential while in the custody of the Office of Victim Services or a victim compensation commissioner.
