§ 33-1005 — Forms. Mailing address.

(a)The Secretary of the State may prescribe and furnish on request forms for:
(1)An application for a certificate of existence;
(2)a foreign corporation's application for a certificate of authority to conduct affairs in this state;
(3)a foreign corporation's application for a certificate of withdrawal; and (4) the annual report. If the Secretary of the State so requires, use of these forms is mandatory.
(b)The Secretary of the State may prescribe and furnish on request forms for other documents required or permitted to be filed by sections 33-1000 to 33-1290 , inclusive, but their use is not mandatory.
(c)If a corporation or a foreign corporation so requests in writing, the Secretary of the State shall mail to the address designated in such request, and to no other address of the cor
