Text

An application for a grant shall be on a form prescribed by the commissioner and shall include, but not be limited to, the following:

(1)A summary that has:
(A)The name, address, telephone number, tax status, federal employer identification number and state tax number of the applicant;
(B)the name, title, address and telephone number, of the applicant's designated contact person;
(C)a description of the principal business of the applicant and the period of time in business in this state, if such period is less than five years;
(D)the number of employees and gross income of the applicant for the preceding year; and (E) a summary description of the project that includes the location and a description of the need for the project and of its benefits to the community, an indication of the
