No provider shall offer or enter into a continuing-care contract in this state or with any resident of this state or regarding any facility in this state and no change in ownership of such a facility shall be completed unless the provider or proposed owner, as the case may be, has (1) registered with the department by filing (A) a current disclosure statement that meets the requirements of section 17b-522, (B) financial information as required pursuant to section 17b-527, and (C) a sworn statement of the escrow agent to the effect that the escrows required by sections 17b-524 and 17b-525 have been established;
