(a)As used in this section:
(1)“Collaborative Care Model” or “CoCM” means the integrated delivery of behavioral health and primary care services by a primary care team that includes a primary care provider, a behavioral care manager, a psychiatric consultant and a database used by the behavioral care manager to track patient progress;
(2)“CoCM codes” means a billing system developed by the Centers for Medicare and Medicaid Services that provide Medicare rates for services provided in the Collaborative Care Model; and (3) “HUSKY Health” has the same meaning as provided in section 17b-290 .
