Text
(1)The board may issue a provisional
license to practice as a physical therapist to a person who:
(a)Submits an application and pays a fee as determined by the director; and
(b)Successfully completes a physical therapy program that meets the
educational requirements in section 12-285-110 (1)(a).
(2)A person who holds a provisional license may only practice under the
supervision of a physical therapist actively licensed in this state.
(3)A provisional license issued pursuant to this section expires no later than
one hundred twenty days after the date it was issued. A provisional license may
only be issued one time and is not subject to section 12-285-114.
