Text
(1)The applicant shall take and submit
test scores from the board-approved exam. The examination shall be of such a
character as to test the qualifications of the applicant to practice optometry.
(2)Each person who makes a passing grade on the practical and clinical
examination and who is otherwise qualified shall be granted a license signed by the
board. The license provided for in this section shall be in such form and wording as
may be adopted by the board. The optometrist shall display the license for viewing
by patients, as provided in section 12-275-117. An application for initial licensure as
an optometrist shall be accompanied by a processing fee in an amount to be
determined by the board pursuant to section 12-20-105.
