Text
(1)A naturopathic doctor shall
provide the following information in writing to each patient in a format required by
the director:
(a)The naturopathic doctor's name, business address, and telephone
number;
(b)The nature of the services to be provided;
(c)A statement that naturopathic doctors are registered by the state to
practice naturopathic medicine under the Naturopathic Doctor Act;
(d)The prohibitions specified in section 12-250-106 (2);
(e)The states in which the naturopathic doctor holds an active license or
registration; and
(f)How to file a complaint against a naturopathic doctor.
(2)A naturopathic doctor shall obtain a written acknowledgment from the
patient stating that the patient has been provided the information described in
subsection (1) of this section
