§ 12-220-405 — Application for dental hygienist license - fee
(1)Every person
who desires to qualify for practice as a dental hygienist within this state must file
with the board:
(a)An application for a license, in a format approved by the board, on which
application the applicant shall list:
(I)Any act the commission of which would be grounds for disciplinary action
under section 12-220-201 against a licensed dental hygienist; and
(II)An explanation of the circumstances of the act; and
(b)Satisfactory proof of graduation from a school of dental hygiene that, at
the time of the applicant's graduation, was accredited, and proof that the program
offered by the accredited school of dental hygiene was at least two academic years
or the equivalent of two academic years.
