§ 12-105-108 — Application - form
(1)Each applicant for examination shall file
with the director a written application in the form the director may require to set
forth the qualifications of the applicant and shall submit satisfactory proof of the
required age and education.
(2)Each applicant for registration shall file with the director a written
application in the form the director may require pursuant to section 12-105-112.
