(a)
(1)The department shall establish the manner and format for emergency medical transport providers to report the data required pursuant to this section.
(2)The department may establish an Internet Web site for the submission of reports required by this section.
(3)The department may require a certification by each emergency medical transport provider under penalty of perjury of the truth of the reports required under this section. Upon written notice to an emergency medical transport provider, the department may impose a penalty of one hundred dollars ($100) per day against an emergency medical transport provider for every day that an emergency
medical transport provider fails to make a report required by this section within five days of the date upon which the report was due.
