A. Each application to participate in a voluntary remediation program shall contain the following information: 1. The name and address of the person filing the application and the nature of the relationship of the person to the site, if any. 2. A description of the location and boundaries of the site or portion of a site that is the subject of the remediation. 3. A description of characterization performed at the site or portion of the site, if known. 4. A general description of the work to be performed or already performed by the applicant. The application shall state whether the applicant is requesting one or more of the following:

(a)Approval of remedial actions to be performed.
