§ 35-1-409 — Form of certificates

(a) In order to promote and maintain uniformity of the
system of vital statistics the forms of the certificates,
reports and other returns required by the act or by regulations
adopted hereunder, shall include as a minimum the items
recommended by the federal agency responsible for national vital
statistics subject to approval of and modification by the
department of health. Social security numbers, if available,
will be required on death certificates.
(b) Each certificate, report and form required to be filed
under this act shall have entered upon its face the date of
registration duly attested.
