Text
(1)A petition for termination of parental rights shall include, to the best information or belief of the petitioner:
(1)(a) the information required by Utah Rules of Juvenile Procedure, Rule 17;
(1)(b) the grounds on which termination of parental rights is sought, in accordance with Section 80-4-301; and
(1)(c) the names and addresses of the individuals or the authorized agency to whom legal custody or guardianship of the child might be transferred.
(2)The petitioner shall attach a copy of a relinquishment or consent, if any, previously executed by the parent or parents to the petition described in Subsection (1).
