Text

(1)When an individual is arrested without a warrant for an offense and booked at a jail facility, an employee at the jail facility, or an employee of a pretrial services program, shall submit the following information to the court with the probable cause statement:
(1)(a) identification information for the individual, including:
(1)(a)(i) the individual's legal name and any known aliases;
(1)(a)(ii) the individual's date of birth;
(1)(a)(iii) the individual's state identification number;
(1)(a)(iv) the individual's mobile phone number;
(1)(a)(v) the individual's email address; and
(1)(a)(vi) the individual's immigration status, if the individual is not a United States citizen or national;
(1)(b) the individual's residential address;
